From legal documents to hospice coordination, we help you take control of your care and plan with confidence.

  • Our 54 page Comprehensive Future Planning Packet that includes advanced care planning resources, guides, and templates.
  • A summary of your advanced care preferences for family and care providers.

Fees

Flat Fees

  • $150/hr for office hours advocacy such as chart review
  • $175/hr for real time advocacy such as in person and virtual meetings and appointments.
  • $75/hr for travel time

Discounts

  • Family Plan Discount: When we work with more than one family member, save 15% per additional person.
  • Frequent Support Savings: Prepay for 10+ hours and save 10%; prepay for 20+ hours and save 15%.
  • Loyalty Reward: Returning clients receive a 5% discount on all future services

Additional Fees

  • $150 per day for expedited summary services (summaries are contracted to take 5 business days)
  • 1.5x rate for after hours work (between 5pm and 8am CST) and weekends (between 5pm Friday evening and 8am Monday morning CST)
